Before You Write Operator Error

'Operator error' is the most common root cause in maintenance — and the laziest. Before you write it, run five checks: check the asset (what actually failed), check the history (what did the data know), check the pattern (did it repeat), check the fix (did retraining solve it), and fix the system (make thinking easier than blame). Blame closes reports. Thinking prevents repeats.
